Medical records do not follow the patient between providers
Problem details
Change city, change insurer, or get referred to a specialist, and your history restarts. Tests get repeated, allergies get re-asked, and the same scan is done twice in eight weeks because one system cannot read the other's format.
Patients are the only party present at every appointment, and they are the one party with no copy of the record.

Moved countries with a chronic condition and effectively started from zero. Repeated a scan I had done eight weeks earlier because the file was on a disc in a format the new hospital could not open.
